I need your help!

So I got my IRB approval. I have my survey done (an abbreviated version of it, didn't realize survey monkey limited you to ten questions). Want to help me and my research student?
We are asking for volunteers with diabetes (type I or type II), hypertension/coronary artery disease, Celiac’s disease and/or breast cancer to take a quick, completely anonymous survey online to assess how much nutritional education and counseling you have received from your medical providers.
It is well-known that nutrition plays a role in managing chronic disease, including the aforementioned conditions. In this survey, we are studying how much or how little information was provided to you by a medical provider. This will help identify strengths and weaknesses which could be used to help modify patient education practices, which could lead to better management of the aforementioned chronic illnesses.
Participants must be 18 years or older. THANK YOU!! Feel free to share with anyone you think would be interested.
